Coordinating subsidised support workers for aged care and NDIS participants and engaging volunteers to strengthen community care.

Providing assistance to access transport support and medical triage services during non-sociable hours to ensure timely access to care.

Our Approach

By working collaboratively with health professionals, community partners, and support networks, we aim to create integrated and effective pathways to care.
